Preparing for Your Appointment:

If you have access to a printer, please print and complete forms (available on www.moimd.com) prior to your visit; this will expedite your check-in.  If you are a new patient, please arrive 15 minutes prior to your scheduled time to take care of registration. 

Bring copies of medical records and studies, including X-rays, MRIs, nerve testing, etc.  It will help if you bring the films (or CD of the films) themselves, not just the reports.  Often you will need to give some notice when requesting films from a facility; please allow yourself some time for this.

Dr. Rohde is a partner of Michigan Orthopaedic Institute, P.C., which is a very large practice; the staff deals with numerous insurance companies, all with different policies and coverages.  Please contact your insurance company to determine your insurance benefits and to inform us of any special requirements.
